WOUND HEALING
Definition of wound:-
A wound is a disruption in the normal continuity of tissue, caused by the transfer of energy into the body. It can affect visible structures like the skin or deeper tissues such as muscles, tendons, or internal organs.
Wound Healing:-
➡ Wound healing is a complex biological process aimed at restoring normal tissue continuity.
➡ It involves a sequence of events, including cellular proliferation and remodeling.
➡ The process typically begins immediately after injury and consists of four
phases in all wounds under normal conditions.
Phases of Wound Healing:
1. Coagulation Phase:
➡ First phase, initiated immediately after injury.
➡ Features vasoconstriction, clot formation, and the release of platelets and healing-promoting substances.
➡ Serves as a bridge between the wound edges.
2. Inflammatory Phase:
➡ Occurs from the time of injury up to three days post-injury.
➡ Involves a classical inflammatory response.
➡ Features vasodilation, fluid exudation, migration of inflammatory cells and leukocytes, and rapid epithelial growth.
3. Proliferative Phase:
➡ Begins around the third day after injury and lasts approximately three weeks.
➡ Marked by events crucial for wound healing:
➡ Proliferation of fibroblasts, epithelial cells, and endothelial cells.
➡ Synthesis of collagen.
➡ Production of ground substance and blood vessels.
4. Maturation Phase:
➡ Also referred to as the remodeling phase, this is the longest-lasting phase, lasting up to one year.
➡ Involves equilibrium between protein synthesis and degradation.
➡ Characterized by cross-linking of collagen bundles, resulting in a gradual increase in tissue strength until it returns to normal.
Clinical Types of Wound Healing:
1. Healing by First Intention:
➡ Occurrence:
This type of healing occurs in clean wounds that are primarily closed soon after the injury, bringing the wound edges together.
➡ Healing Process:
Healing primarily takes place through epithelialization.
Epithelial cells at the wound edges migrate across the wound gap, eventually covering it entirely.
➡ Scar Formation:
First intention healing typically results in minimal scarring, as the wound edges are neatly aligned, and there is less tissue loss to repair.
2. Healing by Second Intention:
➡ Occurrence:
This type of healing occurs in wide or contaminated wounds that are left open, either intentionally or due to the nature of the injury.
➡ Healing Process:
Healing by second intention involves multiple steps:
➡ Granulation Tissue Formation:
Initially, the body forms granulation tissue, which is a type of connective tissue rich in blood vessels. This tissue fills the wound.
➡ Tissue Contraction:
Myofibroblasts, a type of cell, help contract the wound. This reduces the size of the wound but can lead to some deformity or scarring.
➡ Epithelialization:
Finally, epithelial cells at the wound edges begin to migrate and proliferate, gradually covering the wound.
➡ Scar Formation:
Healing by second intention tends to result in more noticeable scarring compared to first intention healing due to the extended healing process.
3. Healing by Third Intention:
➡ Occurrence:
Healing by third intention occurs when a wound is initially left open for various reasons, such as contamination or the need for further assessment and management, and is then closed later, often referred to as "delayed primary closure."
➡ Healing Process:
The healing process in this type combines elements of both first and second intention healing. Initially, the wound is managed as an open wound (similar to second intention healing). Once it is deemed safe and appropriate, it is closed surgically (similar to first intention healing).
➡ Scar Formation:
The extent of scarring in healing by third intention can vary depending on factors such as wound size, contamination, and the timing of closure. It may result in scarring similar to second intention healing, or it may approach the minimal scarring seen in first intention healing.
Factors Affecting Wound Healing:
Local Factors:
➡ Ischemia and Decreased Oxygen Tension:
Inadequate blood supply and oxygen can impair healing.
➡ Presence of Foreign Bodies:
Foreign objects can hinder the healing process.
➡ Closure Under Tension:
Wound closure under tension can delay healing.
➡ Infection:
Infections can significantly impede the healing process.
➡ Irradiation:
Radiation therapy can negatively impact wound healing.
Systemic Factors:
➡ Systemic Diseases:
Conditions like diabetes, cirrhosis, renal failure, and malignancy can impair healing
➡ Poor Nutritional State:
Deficiencies in protein, vitamins, and minerals can slow down healing.
➡ Decreased Resistance:
Immune suppression, chronic infections, and weakened immune systems can hinder healing.
➡ Drug Therapy:
Medications like steroids and cytotoxic agents can negatively affect wound healing.
